Before you make your first purchase with us, we require a few pages of intake paperwork. You can either print them out and bring them with you on your first visit, or you can fill them out in the lobby at your first visit.

Patients Visiting from Out-of-State and Canada

GraniteLeaf Compassionate Care Program
Patients with low income (at or below 200% of the Federal Poverty Level) can apply for our Compassionate Care Program to recieve an additional 10% discount on GLC purchases.
