Therapeutic Cannabis for Parkinson’s Disease

Cannabis has attracted increasing attention as a potential therapy for symptoms of Parkinson’s disease (PD), though research is still ongoing. Parkinson’s is a neurodegenerative disorder characterized primarily by motor symptoms (tremors, stiffness, slowness, and balance issues) and also by non-motor symptoms like pain, anxiety, and sleep disturbances. Many people with Parkinson’s Disease have found cannabis products to be helpful for their various symptoms.

How Cannabis May Help People with Parkinson’s Disease:

Motor Symptom Relief

  • Tremors and rigidity: Some small studies and anecdotal reports suggest that cannabis, especially with CBD or THCA, may help reduce tremors and muscle rigidity.
  • Dyskinesia: Cannabis might help reduce levodopa-induced dyskinesia (involuntary movements caused by long-term use of Parkinson’s medications).

Non-Motor Symptom Relief

  • Sleep: Cannabis, especially products with THC and CBN, may help improve sleep quality, reduce REM sleep behavior disorder (RBD), and ease insomnia.
  • Pain: Many PD patients suffer from chronic pain, and many different cannabis products have shown promise as an analgesic.
  • Anxiety and depression: CBD in particular, has shown potential to reduce anxiety without psychoactive effects, which is useful in managing mood symptoms in PD.
  • Appetite and nausea: Certain cannabis products may help stimulate appetite and reduce nausea, side effects that sometimes accompany PD medications.

Compounds That May Be Beneficial

Cannabinoids

  • CBD (non-intoxicating) – Reduces anxiety, inflammation, muscle rigidity, pain, and tremors.
  • CBG (non-intoxicating) – Increases appetite, provides neuroprotection, reduces nausea, and relieves pain
  • CBN (mildly-intoxicating) – Aids sleep, provides neuroprotection, relaxes muscles, and relieves pain
  • D9 THC (intoxicating) – Aids sleep, alleviates depression, eases anxiety, increases appetite, reduces nausea, relaxes muscles, and relieves pain
  • D8 THC (mildly-intoxicating) – Alleviates depression, eases anxiety, increases appetite, provides neuroprotection, reduces nausea, and relieves pain
  • THCA (non-intoxicating) – Provides neuroprotection, reduces inflammation and nausea

Terpenes

Terpenes are aromatic compounds found in cannabis (and many other plants) that contribute to the unique effects of a specific cultivar (aka strain) of cannabis. The terpenes found in cannabis may be beneficial for relieving the different symptoms of PD. If you want to target specific symptoms, select cannabis cultivars with the terpenes most likely to help with those symptoms.

Risks and Considerations

  • Cognitive side effects: THC may impair cognition, especially in older or cognitively impaired PD patients.
  • Dosing: There’s no standardized dosing, and responses can vary widely between individuals. Start any new cannabis product, especially with THC, at a low dose and increase dosage slowly
  • Drug interactions: Cannabis can interact with Parkinson’s medications, especially those metabolized by the liver. Please consult your health care provider with questions about drug interactions.
  • Legal issues: Before using cannabis products, make sure you know the laws regarding possession and use in your area.

Current Evidence and Research

While some small studies and surveys show promising results, high-quality clinical trials are still limited. Organizations like the Parkinson’s Foundation advise cautious optimism and encourage further study before cannabis is recommended as a standard treatment. Many people with PD find cannabis beneficial for their symptoms when they find the right cannabis product and dosage.
